How to Add Locations When Sending a Form

You can distribute your fillable forms to recipients directly from the Form Templates List with locations.

Navigate to the menu bar icon and click Smart Forms under Quality + Safety.



Select Templates and you will see a list of forms you currently have access to.



Click on the send icon on the form you would like to distribute.


Click on the Location dropdown and select your desired location(s).

Info
Only recipient and description is required to send notification.



Click Send notification once you have filled out the details.
