1. From the form builder panel, click Image
2. Click on the Insert Image Icon
3. Open the image location folder, select - open
4. Click on the tick button to save the image
How to embed Video Form Template
1. From the form builder panel, click Video
2. Click on the Insert Video Icon
3. Open the image location folder, select - open
4. Click on the tick button to save the video embedded
